Job Description

Position: Clinical Pharmacist

Overview

We are seeking a Clinical Pharmacist to provide hands-on patient care, optimize medication therapy, and support providers in improving health outcomes. This role focuses on chronic disease management, patient-centered care, and collaboration with interdisciplinary teams. Care is delivered virtually, in-office, or at patients’ homes. Outpatient experience is highly sought after, and candidates with hospital, ambulatory, or health plan clinical experience are encouraged to apply.

Key Responsibilities

  • Conduct comprehensive medication reviews and develop follow-up care plans with patients, providers, and care teams
  • Assess and optimize medication therapy for safety, efficacy, and adherence
  • Collaborate with interdisciplinary teams to set individualized therapy goals
  • Educate patients on chronic disease management and appropriate medication use
  • Deliver patient care under collaborative practice agreements, including refill approvals and disease-state management
  • Participate in quality improvement initiatives and program development
  • Serve as a trusted resource for medication information and pharmacy benefits
  • Provide professional, timely, and empathetic communication with patients and colleagues
  • Travel to multiple sites as needed (if applicable)

Qualifications & Requirements

Must-Have:

  • Pharm.D. degree or equivalent education in pharmacy
  • Active, unrestricted pharmacy license in TX, or eligibility to obtain one
  • Bilingual in Spanish (high-level)
  • Clinical pharmacy background (no retail experience)
  • Hands-on experience managing patient medication plans
  • Open to new graduates with 1–2 years of residency
  • 3–5 years of relevant clinical experience preferred

  • Experience specifically in primary care
